Why most cycling pain is a setup problem
Cycling fixes the body on three contact points (hands, sit bones, feet) and repeats the same motion five to six thousand times an hour. Any geometric error at a contact point gets amplified by those thousands of repetitions into overuse pain. That's also the good news — if geometry caused it, adjusting geometry can fix it. International research (Priego Quesada et al., 2019) surveying many cyclists found neck and low back are the most commonly reported pain sites, closely associated with bike-fit factors; systematic reviews and clinical commentaries on low back and hip pain likewise find setup changes markedly improve symptoms.
General adjustment rules
- Big before small: set the big geometry (saddle height, front-end height/reach) first, then small parts (cleats, saddle tilt);
- One variable at a time: saddle height ±3mm, fore-aft 5mm, cleat fore-aft 5mm / rotation 2°, ride 2–3 times to evaluate;
- Chain recalibration: after changing saddle fore-aft or cranks, recalibrate saddle height by BDC knee angle;
- Use angles, not feel: knee, hip and torso angles are measurable — don't go by "feels about right."

Is cycling pain normal or a bad fit?
Mild adaptation soreness is normal; recurring, mileage-worsening, location-specific pain is usually an adjustable setup problem. Don't endure numbness.
How to tell which setting from where it hurts?
